Upon admission, you or your loved one will be assessed and screened by every department and an individual plan will be put in place. A care plan meeting will be scheduled with the family and the interdisciplinary team. This meeting allows us to review the goals in the care plan and answer any questions either party has. This is also the time where you and your loved ones can meet the team.
