Upon admission, each resident is assessed and screened by every department and an individual plan is put in place. A care plan meeting is then scheduled with the family and the interdisciplinary team. This meeting allows us to review the goals in the care plan and answer questions either party has. This is also the time where the resident and family can meet the team.
